The Life's WORC At-Home Services Department is designed to provide supports for individuals who reside with their family and require assistance in the development of independent living skills. Working collaboratively with the individual and their selected team or circle, areas of support are identified where a person wants to enhance levels of independence. Areas typically targeted include but are not limited to: personal hygiene or grooming, money management, use of public transportation, food preparation and social skills. The particular choices are then prioritized according to the personal preferences and skill levels of the individual.
Once the areas are identified, the individual in collaboration with their team, which consists of their family members, At-Home Support Staff, At-Home Coordinator and Medicaid Service Coordinator, develop the Individualized At-Home Residential Habilitation Plan. This plan is implemented by the At-Home Care Support Staff each time they visit the person in their home. The goal is to empower the individual and assist them in achieving their identified target areas for improvement or independence.
