Life's WORC: Planned Care Support Services

PCSS ( Planned Care Support Services ) is another HCBS WAIVER service that is provided to individuals who do not require intensive Medicaid Service Coordination and who are eligible for Medicaid. Individuals must be enrolled in Medicaid Service Coordination for at least 3 months and receiving a waiver service, and then can apply to PCSS. The Service Coordinator plays a pivotal role for this service. The individual must meet with his/her Service Coordinator every 6 months to review their service plan and their Waiver Services. Being enrolled into this service allows the individual to maintain his/her Waiver services, such as Residential Habilitation, Day Habilitation, Supported Employment, Respite, etc.
