Life's WORC offers one point of entry to our continuum of services and supports through the efforts of our Social Work Admissions staff. Information about each of the various programs and residential opportunities offered through Life's WORC is available upon request. When a referral packet is received for a program, support or service with our agency, the individual is placed onto our formal Wait List for future consideration.
The Social Work staff will generally arrange for a meeting with the individual, family, Medicaid Service Coordinator(MSC) or program staff to complete an interview to determine eligibility and discuss current or future opportunities. The necessary documentation relating to medical, health or program issues are collected and reviewed by the Agency Admissions Committee. The Social Work staff will maintain ongoing contact with the family or Service Coordinator until the individual is accepted for participation in a home or program of their choice.
The Social Work staff also provide supports to families prior to and after admission into one of our homes or programs. They ensure that all required paperwork, consents, and pre-admission packets are complete. The staff also offers pre-admission and post-admission transition groups for families involved with residential programs.
