Seminar Description: The Front Door is OPWDD’s person-centered approach for people with developmental disabilities that prioritizes individual choices, needs, and desires in making decisions.

During this process, initial eligibility for services will be determined. An assessment of the person’s strengths and needs will be done in order to identify and approve appropriate supports to better connect individual needs to available services. Part of this process also includes participation in an education session, which is required prior to implementation of services. It also assists families in guiding them to where to get the services most appropriate.

The seminar will feature expert speakers and a formal question and answer session.

Immediately following the seminar, please join us for a family networking event over coffee and light snacks.



What is the Family Resource Committee?
Life’s WORC Family Resource Committee is a group of family members and employees who work together to host and facilitate seminars and workshops for special needs families.

If I cannot attend in person will materials be available online?
Yes, the seminar will be recorded and the video will be posted on the Life’s WORC website. Any handout materials will also be made available online after the seminar.

Will childcare be provided?
Yes. Childcare will be available on-site. RSVP prior to the event is required for childcare.

What are my transportation/parking options for getting to and from the event?
A free municipal parking lot is available off of 14th Street and Kellum. Limited street parking is available on 15th Street and Kellum Place.

The LIRR Mineola Station is also within walking distance of the building.

Transition to Care Coordination

As you are aware, beginning July 1, 2018, the former Medicaid Service Coordination program will be transitioned to the Care Coordination Organization/Health Home (CCO/HH) program.

Life’s WORC will be partnering with the Advance Care Alliance, which will oversee program management for our existing clients from July 1 on.

Care Coordination Organization/Health Homes (CCO/HHs) will combine services for developmental disabilities with health, wellness, and behavioral health services to create a single, integrated and individualized Life Plan. A Care Coordination Organization/Health Home (CCO/HH) is a Health Home that is tailored to meet the needs of individuals with I/DD. The CCO/HH will provide care management and coordination services that are tailored specifically to help people with I/DD and their families coordinate all services. CCO/HHs work with individuals with I/DD and their families to bring together health care and developmental disability service providers to develop an integrated, comprehensive care plan (known as a “Life Plan”) that includes health and behavioral health services, community and social supports, and other services. CCO/HHs will assist individuals and their families with accessing services that support healthy, well–rounded and fulfilling lives.

Marianne A. Simoni, Esq.

Andrew Levey is a 25 year veteran of the financial services world, and a Financial Representative with Forest Hills Financial Group. Paulette Elbers

Ms. Elbers is the Trust Administrator at Life’s WORC Trust Services, a role she has held since 2011. As Trust Administrator she oversees the creation and management of special needs and pooled trusts to ensure that our beneficiaries’ needs are met, and their expectations exceeded. Paulette was instrumental in developing the Life’s WORC Trust Program in 2007, now a multimillion dollar program supporting several hundred participants statewide.
